1. Description of Service
Pacific Crest Services provides access to insurance carrier appointments for licensed insurance agents (“User”, “You”, “Your”). By subscribing to our Service, you gain the ability to request appointments with insurance carriers. However, approval for appointments is not guaranteed and is subject to the carrier’s review of the information you provide.

2. User Eligibility
To use the Service, you must be a licensed insurance agent and provide accurate, current, and complete information as requested by Pacific Crest Services. Failure to provide such information may result in denial of an appointment request or termination of access to the Service.

3. Appointment Requests
When submitting a request for an insurance carrier appointment, you acknowledge that:

• Approval is at the sole discretion of the carrier, and Pacific Crest Services does not guarantee that your request will be accepted.
• The information you provide must be accurate and truthful. Failure to meet the qualifications set by the insurance carrier may result in a denial of your request.

4. Subscription Fees and Payment
Our Service operates on a subscription basis. The subscription fee is $350 per month, recurring until canceled by the User or terminated by Pacific Crest Services. You authorize Pacific Crest Services to charge this recurring fee to the payment method you have provided.

• Payment Terms: All subscription fees are billed in advance on a monthly basis.
• Cancellation: You may cancel your subscription at any time. However, no refunds will be issued for the remainder of the billing cycle.
• No Refunds: Once a payment has been made, no refunds will be issued, except in cases where the subscription is canceled prior to the next billing cycle.

5. Privacy and Data Collection
We collect and store personal and professional information as part of the appointment request process. This may include, but is not limited to, your name, contact information, licensing details, and other data as required by insurance carriers.

• Data Use: Your information is used solely for the purpose of securing appointments with insurance carriers and complying with legal and regulatory requirements.

• Third-Party Disclosure: We may share your information with third-parties, such as insurance carriers, in order to facilitate your appointment request.

By opting in to receive SMS messages from Pacific Crest Services, Inc, you agree to receive conversational messages.

• Message frequency: Message frequency may vary. On average, 1-2 messages per month.

6. Termination of Service
Pacific Crest Services reserves the right to suspend or terminate your access to the Service at any time, with or without cause, and without notice. In the event of termination, you remain responsible for any fees incurred prior to the date of termination.

7. Limitation of Liability
To the maximum extent permitted by applicable law, Pacific Crest Services shall not be liable for any indirect, incidental, special, consequential, or punitive damages, including but not limited to loss of profits, data, or other intangible losses, arising out of or related to your use of the Service or inability to secure an insurance carrier appointment.

8. Modifications to Terms and Service
We reserve the right to modify these Terms at any time. Any changes will be posted on the Site and will take effect immediately. It is your responsibility to review the Terms periodically for updates.

9. Governing Law
These Terms are governed by and construed in accordance with the laws of the state of Idaho, without regard to its conflict of law principles.
